Abstract: 

Background: HAND ECZEMA is one of the most common dermatologic diseases requiring treatment but common therapeutics such as corticosteroids and anti-histamines have numerous side effects.So, use of herbal agents, which generally have no major side effects, may be useful especially if their efficacy is established.Malva Sylvestris (MS) is a tropical plant in Iran with cooling characteristics. Hence, in this study, its efficacy in of the treatment of HAND ECZEMA was evaluated.Methods: In this randomized clinical trial, 50 patients with HAND ECZEMA who were visited at Shohada-e Tajrish and Loghman hospitals, Tehran, Iran, between 2006 and 2008, were recruited and divided into two groups. In the first group, MS 4% ointment was applied twice a day and in the other group, placebo ointment was usd. The efficacy and side effects were evaluated three and six weeks after beginning of the treatment.Results: Mean age of the patients was 37.3 years and 35.6 years in MS and placebo groups, respectively (P>0.05). There were no therapeutic adverse effects in the groups. There was a statistically significant difference in all measured scores between two groups in the first and second follow-up sessions (P<0.0001).Conclusion: Malva Sylvestris seems to be a safe and effective therapeutic modality for the treatment of HAND ECZEMA and can be used as an optimal substitute for corticosteroids and antihistamines.

Abstract: 

Background: Various environmental factors can induce or exaggerate HAND ECZEMA. Objective: Evaluation of patch test results in HAND ECZEMA patients. Patients and Methods: In this descriptive study 100 patients with HAND ECZEMA referred to Loghman Hakim & Booali Hospital in Tehran underwent patch testing with 23 allergens obtained from Hennal Company. Results: 240 positive reactions observed in 86 patients. The most common positive reactions were related to nickel sulfate (30 patients, 13.5%) and potassium dichromate (28 patients, 11.7%). In positive group 28 patients were male and 58 were female. In negative group 3 patients were male and 11 were female (P>0.05). The mean age was 31.7±11.9 years and 24.8± 9.2 years for positive group and negative group, respectively (P<0.05). In positive patch test group 49 patients had subacute and 37 patients had chronic ECZEMA. In negative patch test 11 patients had subacute ECZEMA and 3 patients had chronic ECZEMA (P>0.05). Conclusion: Performing patch test, the allergens involved in HAND ECZEMA can be detected. Detecting type of allergens, patients will be able to avoid those materials which can lead to improvement of disease.

Abstract: 

Background & Objective: HAND contact dermatitis, with a one-year frequency of up to 10% and a lifetime prevalence of nearly 15% in the general population, typically involves inflammation of the dermis and epidermis layers .The aim of this study was to investigate and emphasize the anti-inflammatory properties of flaxseed oil as well as its historical use in traditional medicine, which could help support its potential effectiveness in treating HAND ECZEMA and improving the quality of life of patients.  Materials & Methods: The research conducted was a triple-blind, randomized controlled trial involving 68 patients diagnosed with mild to moderate ECZEMA. The participants were assigned to receive either 1 gram of topical flaxseed oil applied twice daily or a control cream, Eucerin, over a period of four weeks. The effectiveness of the treatment was evaluated by measuring the severity of ECZEMA and the quality of life of the patients before and after the intervention. This assessment was carried out using the HAND ECZEMA Harshness Index (HECSI) and the Dermatology Life Quality Index (DLQI). Results:  Based on the gas chromatography (GC) analysis, the flaxseed oil contained 1.9 grams of linoleic acid per 100 grams of oil. There was no substantial variance in the baseline demographic and clinical characteristics of the two study groups. HECSI and DLQI(14.71 ± 1.89) scores significantly improved in the flaxseed oil group compared to the control group (p=0.001). This effect was particularly pronounced among patients with mild and moderate ECZEMA (p<0.001). Conclusion:  Flaxseed oil can improve ECZEMA indications and quality of life in contact dermatitis patients. Abstract: 

Background and aim: Topical glycocorticoid therapy is the first choice for the treatment of ECZEMA. ECZEMA is the most common inflammatory skin disease. Different degrees of itching is the outstanding feature of this disease. In this study we have tried to investigate the efficacy of herbal drug AJMT in comparison with fluocinolone acetonide on HAND ECZEMA.Methods: In this study 64 patients with HAND ECZEMA that referred to dermatology clinic, were randomly divided in two 32 patients group. One group was treated with topical fluocinolone actinide cream and the other with AJMT cream. After 2 weeks of treatment we analyzed the data with SPSS software, using Macnemar and X2 tests.Results: This study showed, there was significant statistical difference regarding to pruritus and erythema in pre and post therapy (P<0.05). The difference was not significant for other symptoms. The results of therapy in AJMT group is superior for symptoms of skin lichenification, scalliness and fissure compared to fluosinolone acetonide group.Conclusion: AJMT cream is more effective than fluosinolone acetonide in HAND ECZEMA. On the other HAND due to more side effects with long usage of topical corticosteroids we prefer the AJMT cream for topical therapy on HAND ECZEMA.

Abstract: 

Background: Palmoplantar psoriasis (PPP) and hyperkeratotic ECZEMA (HPKE) are common and troublesome entities affecting the palms and soles. The diagnosis is made clinically, but differentiation is difficult,histopathology helps but is often impossible in daily practice. Dermoscopy is increasingly utilized to diagnose inflammatory disorders and has been used to differentiate PPP from HPKE. The present study was carried out to evaluate the role of dermoscopy in the differentiation of PPP and HPKE in a tertiary care center of eastern India. Methods: This hospital-based observational study included 20 patients with each of clinically diagnosed PPP and HPKE. Dermoscopy examinations were done, and the results were tabulated and summarized. Fisher’, s exact test was employed to assess statistical significances in differences. Results: Diffuse white and yellow scales were found in both PPP and HPKE without any significant difference. Brown orange globules, clustered dotted vessels, yellow, orange crusts, and perilesional scaling were found more in HPKE (P < 0. 05). On the other HAND, uniformly dotted vessels and background erythema were more characteristic of PPP (P < 0. 05). The results of the current study had deviations from the results of studies done abroad, which can be ascribed to differences in skin color. Conclusion: Dermoscopy is a valuable tool to sort out diagnostic dilemmas in cases of PPP and HPKE. It should be used more commonly to gain more experience and information in the diagnosis of inflammatory dermatoses.

Abstract: 

Background & Objectives: ECZEMA is an inflammatory skin reaction to exogenous and endogenous factors which originate by processes within the body. One of clinical forms is Dry palmar ECZEMA. This condition affects housewives and cleaners who frequently immerse their HANDs in water and detergents. Emollients and topical steroids are used in all cases of ECZEMA. It is believed that alpha-hydroxy acid agents causes the regrowth and increase the viable epidermal thickness. This study showes the effects of Alpha- hidroxy acids and Vaseline on housewives HAND ECZEMA.Methods: At this clinical trial, 22 housewives who had mild dry palmar ECZEMA were entered to study. One HAND was treated by A.H.A (7% glycolic acid) and the other HAND by vaselin (placebo) for a month, four times daily. Collected data were analyzed by SPSS soft ware.Results: Age mean of patients were 26±2 years. There was a significant relationship between Alpha-hydrocy acid and erythema improvement whith 99% confidence (p<0.01) and also between Vaseline and arrhythmia improvement with 95% confidence (p<0.04). There was a significant relationship between performing preventive recommendation and the decline in the number of complaints (p<0.01). Improvement was seen in the epidermal atrophia in 50% of the cases in Alpha-hydroxy acid group (p<0.0001). Whereas in the Vaseline group' it was not seen.Conclusion: Improvement and repair in Damages of stratum corneum of Epidermis by A.H.A agents is better done than through Vaseline. Howere, Vaseline is also effective in decreasing complaints and Erythema.
